How to Release Your Previous Doctor

Follow these quick steps to leave your previous physician in the Florida MMU Registry so you can transfer to BudDocs.

📋 Before you begin: Make sure you have your MMU Registry login ready. Your username is your email address. If you don’t remember your password, click “Forgot Password” on the login page to reset it.

Steps to Release Your Previous Doctor

Log in to your MMU Registry
Go to the Registry login page and sign in with your email and password:

Go to “Manage Information”
Once logged in, click the 3 bars (☰) in the top right corner and select MANAGE YOUR INFORMATION.

Select “Patient Profile”
From the menu, click on Patient Profile to open your profile page.

Click “Leave Physician” and confirm
On your Patient Profile page, click the purple “Leave Physician” button. A confirmation prompt will appear — select “Yes” to complete the process.
💜 The “Leave Physician” button is purple — look for it on your Patient Profile page. Once you select “Yes” to confirm, the process is complete and you are ready to transfer to BudDocs!
